WORKERS COMPENSATION INJURY
IMPORTANT INFORMATION REGARDING YOUR WORKERS COMPENSATION ACCIDENT (W/C)
Listed below is important information regarding the billing and handling of your workers compensation accident claim.
You must notify your employer as soon as possible and request Form 801, “Report of Job Injury or Illness,” complete the “worker” portion of the form, and return it to your employer.
You will need an “Attending Physician” for your claim. You may choose a provider of your choice and complete the “worker” portion of Form 827, which is provided by the provider. This form notifies both your employer and the work comp insurance carrier that you are seeking medical treatment. Work comp only allows you up to three (3) attending physicians for the lifetime of your claim.
Chiropractor & Naturopathic Physicians may only be your attending physician for up to 60 calendar days. Nurse Practitioners are not designated as attending physicians but may treat you for up to 180 days. Your claim will then be referred to a DO or MD physician who will be your attending physician for the remainder of your claim.
The insurance company may enroll you in a managed care organization (MCO) at any time: Majoris, Providence, or Kaiser. Our facility works with Majoris MCO only. The MCO coordinates treatment and helps manage your claim.
If you require an interpreter, your health care provider can assist you in getting this arranged for your visits. The insurer may pay for the services when the interpretation is for an accepted condition. Family and friends cannot be paid to interpret.
You must be seen by your work comp doctor at least every 30 days as required by the work comp insurance carrier, or your claim may be closed or denied.
If you cannot attend an appointment due to lack of transportation, contact your claims adjuster—they may be able to assist.
The work comp insurance carrier has up to 60 days to accept or deny your claim.
The insurer may choose to send you for an Independent Medical Examination (IME). The insurer will choose the IME provider and pay for your exam. They may require you to attend up to three IMEs.
If at any time you choose to have legal representation, you must notify our office and fill out an Authorization to Disclose form so we may communicate with your attorney. Once you choose to be represented by an attorney, only the attorney may communicate on your behalf with the insurance carrier.
Please be advised that Drs. Takacs do not accept protection letters or liens from attorneys.
