IMPORTANT INFORMATION REGARDING YOUR MOTOR VEHICLE ACCIDENT (MVA)

Listed below is important information regarding the billing and handling of your motor vehicle accident claim.

Personal Injury Protection (PIP): For most Oregon claims, there is a $15,000 cap for medical claims. If you use this amount in full, your claim will be denied as PIP exhausted. Most Oregon claims have a two (2) year treating limit. If you treat beyond your accident date, the claim will be denied as PIP expired.


To get PIP benefits, you have to cooperate with your insurance company. You MUST sign a medical release, so they may get your medical records and speak with your doctors. You will need to fill out a PIP application, which is provided by your adjuster.


Please understand that motor vehicle companies only let us know that a claim has exhausted after we submit a claim. They will return the claim to us with a denial stating benefits have been exhausted or expired. Since it can take up to 30 days before we are notified, you may have had further treatment and may incur charges that will not be covered. Insurance companies do not call us when your benefits have exhausted or expired. Likewise, when we initially call your insurance to verify your claim is open, they will only advise us that the claim is open. They are not able to disclose how much PIP has been used or is remaining on your claim. 


In the event your PIP has exhausted or expired, we can work with you should you still require medical care. If you have personal health insurance, we are able to bill them, only after you have contacted them to advise them that you have had an accident. Please check with a staff member to verify our provider(s) are on your insurance plan. Your insurance company will put you in touch with the subrogation department to open a claim. Please be advised, not all insurance companies do this, or there may be a limit on what they pay out for an accident. If you do not have personal health insurance, or do not wish to go through your health insurance, we are able to offer you a cash discount for payment at time of service or we can arrange a payment plan.   • Should your PIP expire, be exhausted, or be denied, monthly payments of $75 are required. Items not covered like natural medications, braces, chart notes, forms, etc. are patients’ responsibility.


Please be advised that Drs. Takacs do not accept protection letters or liens from attorneys. If you have an attorney, please complete an Authorization to Disclose form sw we may share information with them.
