I hope this message finds you well. I want to share some important updates regarding my medical practice that will impact your care.

After much consideration, I have accepted a position as Medical Director at the Brain Rehab Network (BRN) in Portland. This transition means I will be modifying my current practice.

Here’s what you need to know:

1. My last day seeing patients at the 5909 SE Division location will be June 23, 2025.

2. Beginning July 7, 2025, I will be seeing patients at my new office:

3. As of September 1, 2025, I will no longer provide care for injured workers. For those with an active worker's compensation claim, I will continue as your attending physician until August 31, 2025, but your appointments will be at the new location. After that date, you’ll need to find a new physician. If you wish to remain in SE Portland, you might consider reaching out to Drs. Joan and/or John Takacs (503-234-1531) or Integrated Rehabilitation (503-232-1000) to discuss transferring your care. The Saif or Majoris panel website can also be a useful resource for finding a new attending physician. I will assist you in transferring your medical records to your new provider as needed.

4. For commercial insurance and all other non-work-related clients, I will maintain an active practice at my new location and can continue to see you.

5. To request a copy of your medical records or to have them sent to another provider, please email susanschmitt.brn@gmaiI.com with "Request for Records" in the subject line. A request form will be sent to you promptly (usually within three minutes). Please note that a small fee may apply for copies of your records.

I understand these changes may create some challenges for you, and I assure you that my staff and I will do everything we can to minimize any disruption in your care. For those who continue to work with me, I am committed to providing you with the highest quality care. To those who are moving on, it has been a privilege to work with you, and I sincerely thank you for trusting me with your health.
