The office is open Monday through Thursday from 8:00 am to 5:00 pm and Friday from 8:00 am to 12:00 pm.
All office visits, except emergencies, are by appointment. Please make separate appointments for each individual family member. When you call for an appointment, the receptionist will ask the nature of your problem in order to allow an adequate amount of time for your visit. We will make every effort to honor your appointment time. Occasionally, however, emergencies may cause unforeseen delays.
If you cannot keep your appointment, please cancel 24 hours in advance. New patient appointments not cancelled at least 24 hours in advance will be charged $50.00 and established patients will be charged $35.00. This charge will not be billed to your insurance company.
If you are running late, please call the office to let us know. If you are running more than 15 minutes late, we will need to reschedule your appointment.
Please download, fill out, and print the New Patient Forms Packet for Dr. Guyette (ADULT or CHILD) before your visit. This will lessen the amount of time you have to spend at the office. You can fill these out using your computer, or you can print them and fill them out by hand. Bring these forms to your first appointment.
